Many Empire Plan Enrollees received a letter from Empire but were confused by the reference to the co-payment code changes from A to D. Please find below an explanation:
Please note: In December, all Empire
Plan enrollees should be receiving new cards in the mail. The new cards
will have a new co-payment
code. The co-payment
code is being changed from A to D. This means that your medical co-payments
will be going from $18.00 to $20.00 effective January 1, 2009. As of January
1, 2009, you should be using the new card. This code change applies to your
medical co-payments. You should be receiving a publication, shortly, entitled “At
a Glance 2009”, which will inform you of other changes to your plan
